Household Task


Assistance with Travel/Transport

Assistance with travel and transport, to support people access the community where they cannot do so through other means of transport. Transport/Travel Support allows you to use your own vehicle or a support worker's vehicle. Tasks that may be classified as transport could include:

  • Getting to appointments or therapy support;
  • Day-to-day activities;
  • Transport to education or work.
  • social and medical appointments
  • everyday tasks such as shopping, banking etc
  • visits to family and friends, social activities, day trips
  • visits to gym, swimming, library

Development of Daily Living & Life Skills

We will work with you to understand the skills you want to learn or develop. Our main focus is on building confidence and independence to allow you to truly reach your potential. We Empower team will provide all the essential support to develop the skills you want to learn, such as how to cook, use public transport, manage a budget or travel independently. Programs are offered one to one or in a group. Our programs can include:

  • Nutrition- shopping for and preparing healthy food
  • Personal hygiene – Presentation and personal grooming
  • Physical wellbeing – exercise and relaxation
  • Money handling
  • Bill paying
  • Job readiness – resume and interview preparation
  • Household tasks
  • Public transport training
  • Communication and social skills
  • Working with the public trustee
  • Time management
  • Managing medications and medical appointments
  • Self-care
  • Sleeping well
  • Problem solving

Group and Centre Based

Group and Centacare Centre-Based Services support people with a disability to participate in community, social and recreational activities, developing skills and achieving goals through a mixture of one on one or group activities.

Community Nursing

Community Nursing means the nurse comes to you! For many reasons it can be difficult to get to the hospital for regular checkups or for emergencies. We understand this can be much more difficult for people living with a disability and their families. WE Empowers Community nurses can provide assistance after returning from hospital, manage chronic and lifelong illness, offer selected hospital services, administer medication etc.

  • Visiting nurses
  • Medication set-up
  • Medication administration/supervision
  • Continence advice
  • Diabetes management
  • Dementia management
  • Wound care
  • Monitoring of blood pressure, pulse, temperature and blood sugar levels
  • Education for clients and their families
  • Stoma Care

Participate in Community, Social and Civic Activities

Our dedicated team offers participants to engage in community, social and recreational activities. We assist you in connecting with the community and support you to the activities you enjoy and learning new skills. Our program includes:

  • Attend personal development courses
  • Join a social group
  • Take an outing or a holiday
  • Go on a holiday camp
  • Visit your local library
  • Watch a movie or a concert
  • Go to community events, and/or
  • Join a sporting club.

Daily Personal Activity

These supports are provided individually to participants and can be provided in a range of environments, including but not limited to, the participants own home. Assistance with Daily Life Activities gives an NDIS participant the core support they need to maintain their independence and accomplish daily activities. This program may include:

  • Mobility and Hoist transfers
  • Showering or bathing
  • Dressing & Grooming
  • Attending events or planning your day
  • Going to the toilet & Personal hygiene
  • Help with preparation of meals
  • Feeding and conjunction with preparation of meals
  • Moving around the house
  • Dishwashing and cleaning
  • Garden Maintenance
  • Shopping
  • Any general household chores that you may find challenging
